Our admissions team is ready to help you!

At The Guardian Center, a dedicated admissions team works closely with the resident and their family members to assist in processing all aspects of admission. Our facility is prepared to accept new admissions 24 hours a day, seven days a week.

The goal of the admissions team is to ensure collaboration between various care continuums during the admitting process.

Steps in this process are thoroughly explained to alleviate the stress that can accompany changing healthcare settings. Once admitted, our team of clinicians, in unison with the facility’s Medical Director, will evaluate and craft an appropriate plan of care.

Specific and measurable objectives and goals are outlined, and every aspect of a resident’s needs are assessed. For the duration of the resident’s stay, Social Services will maintain close contact with both the resident and family members.

Social Services will focus on providing excellent care and unmatched satisfaction to the resident throughout their stay at The Guardian Center.
